This is a baseball shirt from a sports store in Toronto called "Unique Crests Athletic Supplies LTD".
The tag literally translates to "Unique Crest"?

I just want a simple plain baseball shirt. It has a single zipper.
It's made of a thin, sturdy wool fabric with a green stripe.
It's like a vest, so you can wear it over a shirt or T-shirt.
